Tax Form

This is a multi-state Sales Tax Exemption Form. Not all states allow all exemptions that are listed on this form. Purchasers are responsible for knowing if they qualify to claim exemption status from sales tax in the state that the taxes are due on the sale. Please download and fill-out the Tax Form completely
